Sponsor Form

Sponsor Sheets are available only for approved situations.

We will provide one sponsor sheet per family per season.

The sheet must be signed and stamped by the director in order to be honored.

The sheet will include a deadline for all money and information to be turned in.

Any other fundraising, soliciting or falsification will result in permanent removal at BDS.
